License Renewal Information
Renewal forms are mailed to the address we have on file approximately 60 days prior to your expiration date. If you did not receive your renewal form, you may print a replacement form and mail to our office.
Mail your renewal and fees to our office at: 1625 North Market Blvd., Suite N-327, Sacramento, CA 95834.
Fees may be made by personal check or money order payable to: CA Board of Chiropractic Examiners.
